Our all-inclusive 4th trimester Package includes:
A prenatal virtual consult
8 weeks of care in your home with a board-certified pediatrician and a breastfeeding medicine/lactation specialist
All routine pediatric care IN YOUR HOME for your baby’s first 8 weeks -
-5 home visits at a minimum, initial newborn visit
- weight and feeding check-ups, 2-week well-check
- 1-month well-check, 6-8 week “discharge” visit
- additional baby sick visits as needed (virtual or in-home)Unlimited breast/chest/body-feeding or bottle-feeding assistance throughout the 8-week postpartum period
Direct access to your 4th-trimester pediatrician during the 8-week package, by text, e-mail, or phone with frequent check-ins
Maternal postpartum and family support and monitoring (including partner support)
Coordination of care and transfer of care to your chosen long-term pediatrician will be arranged at the end of the 8 weeks, and your baby’s 2-month well-check with be with your long-term pediatrician
Additional visits can be scheduled for an additional fee
There may be additional travel fees based on location (see below)

- 40-60 minutes from River Forest, IL: $80/visit
- >60 minutes - please inquirePlease inquire about fully virtual 4th Trimester Package options, that can provide unparalleled remote support throughout this period if you are in Illinois but not within our service area.
Please also inquire about special pricing for twins or other multiples.
50% of the fee is paid at the time of registration, with the remainder due after the baby’s birth You may pay the entire fee at registration if you prefer. We can set up a personalized payment plan as well upon request.
HOW CAN I PAY FOR MY PACKAGE?

THE LACTATION NETWORK
TLN may be able to pre-approve your benefits for any 4th Trimester visits related to lactation. Blue Cross PPO, Blue Shield PPO, Anthem PPO, Humana PPO, UHC, and VA CCN (Veterans Affairs Community Care Network) plans as well as most plans that utilize MultiPlan and PNOA. This may decrease the cost of our package by as much as $1,000.

REIMBURSIFY
Upon request, we will provide you with a Superbill or receipt with all the information you need to submit your fees to your insurance for possible out-of-network reimbursement. We have partnered with Reimbursify for you to easily submit your claims for out-of-network health insurance reimbursement.

OTHER PAYMENT OPTIONS
We are happy to split your payments up over a longer period of time upon request. You are also able to use HSA/FSA funds to pay for your services. If using your FSA,contact your employer to verify.

Dr. Rubin's fourth trimester package was the best gift I could have possibly given myself in the postpartum period. I just had a whole baby and you want me to leave my house 2 days later? No thanks. Dr. Rubin was able to spend SO much time with baby and me, going over everything from baby's health to extensive breastfeeding consultations. It is really useful to be able to troubleshoot breastfeeding in the place where you'll be doing most of it--at home. My health ended up being relevant as well with low supply and she gladly consulted with that as well. Dr. Rubin was patient, cheerful, compassionate, and non-judgmental. Would highly recommend!
